ACT for Specialized Mental Health Treatment
The Assertive Community Treatment (ACT) Team, made of qualified mental health professionals, is our most intensive service provision team to provide specialized mental health treatment that supports an individual’s ability to remain in the community by identifying and addressing their personal recovery goals.
A person receiving ACT services will receive:
- Medication management services provided by their psychiatric provider in StarCare’s Outpatient Clinic
- Evidence-based psychosocial rehabilitation and medication education & support services provided by ACT staff with the use of the Illness Management and Recovery Curriculum
- In-home medication monitoring and administration weekly, bi-weekly, or daily as determined by the psychiatric provider
To be eligible to receive ACT Team services, people must:
- Be 18 years or older
- Have a diagnosis of Schizophrenia, Bipolar Disorder, or Major Depressive Disorder
- Have a history of psychiatric hospitalizations
