What Can I Expect?
Before calling, make sure you have the following information available specific to the person seeking services.
- Address
- Phone number
- Date of birth
- Social security number
- Medications list
- Proof of insurance/Medicaid/Medicare
Upon completion of the screening, internal or external referrals will be made as appropriate.
How are services paid for?
StarCare accepts Medicare, Medicaid, and most forms of private health insurance.
For persons who are not eligible for health coverage, a discounted/sliding fee schedule is available. Fees are assessed based on proof of the individual’s (or in some cases the family’s) complete income.
No one will be denied services because of an inability to pay.

Eligibility Determination
Screening
Screening can be completed by calling 806-740-1421 or by walk-in at our Sunrise Canyon Complex located at 1950 Aspen, Lubbock, Texas
Please refer to the “What to Expect Box” above to ensure you have the appropriate information available when you call or visit.
ELIGIBILITY DETERMINATION
Based on the results of the screening, an appointment for eligibility determination will be scheduled. Community referrals will be provided when individuals do not meet criteria for eligibility determination.
SERVICE AUTHORIZATION
When eligibility is determined, services will be authorized and the individual will be contacted by StarCare to initiate services. Please note, authorization into services is contingent upon available capacity. In the event the individual is determined ineligible for services, community referrals will be provided.

Intellectual Disabilities
StarCare provides and coordinates intellectual disability (ID) services and assists those wanting placement on the Home and Community-Based Services and/or the Texas Home Living interest lists.
To Qualify for ID Assessment
Individuals Must:
- Live in Lubbock, Lynn, Cochran, Crosby, or Hockley counties
After screening, the Eligibility Department will assess intellectual functioning by conducting a clinical interview which includes:
- intelligent quotient (IQ) testing
- adaptive behavioral screening
- pervasive developmental disorder screening (if needed)
Mental Health Assessment
StarCare coordinates a network of services that includes preliminary diagnosis and determination of a service package with linkage to appropriate services for children, adolescents, and adults.
To Qualify for Mental Health Assessment
Individuals Must:
- Live in Lubbock, Lynn, Cochran, Crosby, or Hockley counties
- Be within the following age ranges:
- Children/Adolescents
- 18 and older
Substance Use Assessment
StarCare provides services for children and adults looking for, and in need of, substance use services and/or support through the Outreach, Screening, Assessment, and Referral (OSAR) Department.
To Qualify for OSAR Screening
Individuals Must:
- Need substance use treatment services (as determined from the screening)
- Live in one of the 41 counties of Region 1
- Be 12 years or older
